Job Summary:
The Physical Therapist (PT) provides comprehensive outpatient home health services to diverse patient populations, focusing on improving mobility and independence. The role involves assessing patient needs, developing personalized treatment plans, and delivering age-appropriate care in a home setting.

Responsibilities:

• Assess patients' physical function and develop individualized care plans to meet their recovery goals.
• Implement therapeutic interventions, including patient transfers and positioning, to enhance mobility.
• Evaluate treatment effectiveness and modify plans accordingly to optimize outcomes.
• Document all patient care activities accurately and promptly in electronic medical records.
• Communicate effectively with patients, families, physicians, and healthcare team members to coordinate care.
• Ensure compliance with all documentation, billing, and coding protocols related to home health services.

Qualifications:

• Hold an active Illinois state license as a Physical Therapist.
• Possess at least 1 year of recent home health physical therapy experience.
• Demonstrate proficiency with OASIS data collection and start-of-care assessments.
• Maintain current BLS certification and active CPR certification.
• Have a valid driver’s license, current car insurance, and reliable transportation.
• Be vaccinated against COVID-19 with proof of vaccination.
