Job Summary:

Provide skilled physical therapy services to patients in a skilled nursing facility, promoting recovery, mobility, and independence. Evaluate patient needs and develop personalized treatment plans to improve functional outcomes.

Responsibilities:

• Assess patients' physical conditions and functional limitations to determine appropriate treatment strategies.
• Develop and implement individualized care plans aligned with physician prescriptions.
• Administer therapeutic exercises, manual therapy, and modalities to facilitate recovery.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and their families on exercises and techniques to support ongoing recovery.
• Document treatment sessions, progress notes, and patient responses accurately in compliance with facility policies.

Qualifications:

• Hold a valid state license or registration as a Physical Therapist.
• Proven experience working in SNF or rehabilitative settings preferred.
• Demonstrate strong clinical assessment and manual therapy skills.
• Possess excellent communication and patient education abilities.
• Maintain current CPR and BLS certifications.
• Ability to collaborate effectively with multidisciplinary healthcare teams.
