Job Summary:
Provides skilled nursing care to residents in a long-term care or skilled nursing facility setting. Collaborates with the healthcare team to ensure comprehensive patient care and safety.

Responsibilities:

• Administer medications and treatments following healthcare protocols
• Monitor residents’ health status and document changes accurately
• Assist with activities of daily living and personal care needs
• Communicate effectively with residents, families, and team members
• Ensure compliance with all regulatory and safety standards during care delivery
• Maintain a clean and safe environment for residents and staff

Qualifications:

• Valid and current LPN license in good standing
• Proven experience in long-term care or skilled nursing facilities
• Strong knowledge of medication administration and resident care procedures
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a healthcare team
• Flexibility to work 12-hour shifts as scheduled
