Job Summary:

Seeking a dedicated Registered Nurse (RN) to deliver high-quality, patient-centered care within a home health setting. This role involves conducting assessments, developing care plans, and providing skilled nursing interventions directly in patients' homes.

Responsibilities:

• Perform comprehensive patient assessments to determine care needs.
• Develop and regularly update individualized plans of care.
• Deliver skilled nursing interventions and monitor patient progress.
• Educate patients and families regarding medications, treatments, and care procedures.
• Collaborate with LPNs, therapists, caregivers, and healthcare providers to coordinate care.
• Complete accurate and timely documentation using EMR systems, preferably HCHB.

Qualifications:

• Current Registered Nurse (RN) license in Colorado or active NLC compact license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong assessment, critical thinking, and independent working skills.
• Effective communication skills for interacting with patients and team members.
• Ability to manage documentation and EMR systems effectively.
• Home health experience is preferred but not mandatory.
