Job Summary:
Seeking a qualified Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide compassionate and efficient care in a long-term care or skilled nursing facility setting. The ideal candidate will demonstrate strong clinical skills and a commitment to resident well-being.

Responsibilities:

• Administer direct patient care according to physician orders and facility protocols.
• Monitor and document patient vital signs, symptoms, and progress.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Ensure compliance with all safety, health, and regulatory standards.
• Educate patients and their families on care procedures and health management.
• Maintain accurate and complete documentation of nursing interventions.

Qualifications:

• Hold a current and valid LVN/LPN license.
• Possess at least 1 year of experience in LTC/SNF or similar settings.
• Complete a state or federal issued identification and wear a photo badge on site.
• Be vaccinated against COVID-19 or qualify for an exemption with proper documentation.
• Demonstrate excellent communication and clinical skills.
• Ability to work effectively in a fast-paced healthcare environment.
