Job Details

Job Summary:
Provide skilled physical therapy services to patients in a home health setting, ensuring quality care and adherence to treatment plans. The therapist will travel to patient homes, perform assessments, and develop recovery programs to improve patient mobility and function.

Responsibilities:

• Assess patients' physical condition and develop individualized treatment plans.
• Deliver therapeutic interventions and techniques tailored to each patient's needs.
• Document patient progress accurately within established timeframes.
• Travel to patient homes within assigned territories, managing a caseload of 5-6 patients daily.
• Collaborate with healthcare team members and participate in periodic office meetings.
• Maintain current licensure, CPR/BLS certification, and agency-required liability insurance.

Qualifications:

• Hold a valid state license as a Licensed Physical Therapist.
• Possess at least 1 year of recent adult home health therapy experience.
• Display proficiency in documentation and electronic medical records systems, specifically HCHB.
• Have reliable transportation, a valid driver's license, and auto insurance.
• Be willing to travel up to 75 miles from the care center as required.
• Demonstrate strong communication and organizational skills.
