Job Summary:


We are seeking a dedicated Physical Therapist to provide high-quality home health services, including patient assessments, treatment planning, and therapeutic interventions. The therapist willtravel to patient homes to deliver personalized care and ensure optimal rehabilitation outcomes.



Responsibilities:

• Evaluate patient conditions and develop individualized treatment plans based on assessment findings.
  
• Perform therapeutic procedures and exercises to improve patient mobility, strength, and functionality.
  
• Document patient progress, treatment sessions, and clinical notes accurately within the EMR system.
  
• Travel to multiple patient homes within the designated territory, adhering to scheduled visits and timelines.
  
• Collaborate with interdisciplinary team members to coordinate care and modify treatment plans as needed.
  
• Maintain compliance with all licensing, certification, and agency policies, including patient safety protocols.
  
  
  
  Qualifications:
  
• Hold a valid state-issued license in Physical Therapy.
  
• Demonstrate a minimum of 1 year of recent home health PT experience.
  
• Possess current CPR and BLS certifications.
  
• Have reliable transportation, a valid driver's license, and liability insurance.
  
• Be proficient in HomeCare HomeBase or similar EMR systems used for documentation.
  
• Ability to travel up to 75 miles from the home clinic to patient locations.
