Job Summary:
Provides specialized wound, ostomy, and continence nursing care to patients through comprehensive assessments, treatment planning, and education. Collaborates with the healthcare team to ensure optimal patient outcomes and infection control standards.

Responsibilities:

• Develop, review, and implement standards of care related to wound, ostomy, and continence management.
• Assess and document patient responses to treatment, adjusting plans accordingly to promote healing.
• Collaborate with multidisciplinary teams to facilitate referrals and coordinate comprehensive patient care.
• Educate patients and families on prevention, management, and self-care strategies for wounds and related conditions.
• Conduct staff education sessions, including orientation, in-services, and ongoing professional development.
• Participate in quality improvement initiatives, data collection, and clinical research to enhance wound care practices.

Qualifications:

• Hold an Associates Degree (ASN, ADN) from an accredited Nursing Program.
• Licensed as a Registered Nurse in New York State with a current NYS RN license.
• Possess a valid AHA BLS certification.
• Have at least two years of Med/Surg nursing experience, including wound and colostomy care.
• Maintain certification as a Wound Ostomy Continence Nurse (CWOCN), CWON, CWCN, CWS, or WCC issued by recognized certifying boards.
• Demonstrate strong clinical assessment, patient education, and collaborative skills.
