Job Summary:
Provides comprehensive outpatient and inpatient social work services to high-risk patients at Tuba City Regional Healthcare Corporation. Responsible for assessment, care planning, resource linkage, and advocacy to promote patient self-sufficiency and quality outcomes.

Responsibilities:

• Assess patient needs and develop individualized care plans focused on community resources and support systems.
• Coordinate care by linking patients with social services, mental health, substance abuse, and other healthcare providers.
• Monitor patient progress, evaluate medical necessity, and review referrals for specialty services.
• Collaborate with multidisciplinary teams to facilitate seamless care and discharge planning.
• Advocate for patients to access appropriate community and healthcare resources efficiently.
• Support patients with mental health issues, assisting them in navigating resources to improve well-being.

Qualifications:

• Hold a Master’s degree in social work (MSW).
• Possess a valid, current LMSW or LCSW license in the United States, Puerto Rico, or U.S. territories.
• Maintain current Basic Life Support (BLS) certification by the American Heart Association.
• Have a minimum of three (3) years of clinical social work experience.
• Demonstrate knowledge of discharge planning, case management, and electronic health record systems.
