Job Summary:
Provides emotional, social, and practical support to patients with chronic kidney disease and their families, helping them navigate the challenges of treatment and disease management. Manages complex psychosocial conditions to promote patient well-being and adherence to care plans.

Responsibilities:

• Assess patients’ psychosocial needs related to chronic kidney disease and dialysis treatment.
• Develop and implement individualized care plans to support emotional resilience and social stability.
• Collaborate with healthcare teams to coordinate discharge planning and community resources.
• Educate patients and families on coping strategies and available support services.
• Document patient interactions and care progress accurately in EMR.
• Ensure compliance with licensing, certification, and institutional protocols.

Qualifications:

• Possess a minimum of 2 years of direct Social Worker experience, with specific dialysis work preferred.
• Hold an active American Heart Association BLS certification.
• Maintain a current state license to practice as a Social Worker in Michigan.
• Demonstrate proficiency in assessment, counseling, and case management skills.
• Exhibit strong interpersonal and communication abilities to engage patients and families effectively.
• Provide references from supervisors within the past two years.
