Job Summary:
Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting. This role involves supporting patient needs and ensuring compliance with healthcare standards.

Responsibilities:

• Evaluate residents' health conditions and monitor vital signs regularly.
• Administer medications and treatments accurately as prescribed.
• Document patient information and care provided in electronic health records.
• Collaborate with healthcare team members to develop and implement care plans.
• Educate residents and families on health management and care procedures.
• Comply with facility policies, safety procedures, and infection control protocols.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF environments preferred.
• Strong understanding of medication administration and patient care standards.
• Excellent communication and interpersonal skills.
• Ability to work a flexible schedule, including alternating shifts and weekends.
• Reliable and dependable with a commitment to patient care.
