Job Description
Job Summary:
Provide skilled nursing care to residents in long-term care or skilled nursing facility settings. Deliver holistic patient care while ensuring compliance with facility policies and healthcare regulations.
Responsibilities:
- Assess patient health status and develop individualized care plans.
- Administer medications and monitor patient responses.
- Collaborate with interdisciplinary teams to coordinate care.
- Document patient information accurately and timely.
- Educate patients and families regarding health conditions and treatments.
- Adhere to safety protocols and infection control procedures.
Qualifications:
- Valid registered nurse license in the state of practice.
- Minimum of one year of experience in LTC or SNF environments.
- Current CPR and BLS certifications.
- Ability to effectively communicate with patients, families, and healthcare team.
- Demonstrate strong clinical assessment and critical thinking skills.
