Job Summary:

Provides coordinated home health nursing care tailored to physicians’ orders, utilizing the nursing process to ensure quality patient outcomes. Supervises and evaluates home health aides, licensed practical nurses, and licensed vocational nurses to maintain high standards of care.

Responsibilities:

• Coordinate and deliver home health nursing services, ensuring adherence to agency policies and physicians’ orders.
• Supervise and evaluate home health aides, LPNs, and VNs to support clinical excellence.
• Conduct patient visits including Start of Care, wound care, and follow-up assessments.
• Complete all documentation accurately within 24 hours of patient encounters.
• Travel up to 75 miles from the home clinic to reach patients, managing a caseload of 5-8 patients daily.
• Participate in office meetings and maintain proficiency with the HomeCare HomeBase (HCHB) EMR system.

Qualifications:

• Hold an active RN license in the state of assignment with no pending licensing issues.
• Possess at least 1 year of recent adult home health or hospice experience.
• Proficient in OASIS documentation if applicable, or willing to complete retraining.
• Demonstrate reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Capable of traveling up to 75 miles and managing a variable patient load.
• Willing to work on a flexible schedule, including every 3rd weekend and on-call rotations.
