Job Summary:
Provide skilled nursing care within a long-term care or skilled nursing facility setting. Ensure patient needs are met through compassionate and comprehensive nursing services.

Responsibilities:

• Assess patients' health conditions and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and document clinical observations accurately.
• Collaborate with multidisciplinary teams to coordinate care delivery.
• Maintain compliance with healthcare regulations and facility policies.
• Educate patients and families about health management and post-discharge care.

Qualifications:

• Hold a valid state or federal nursing license.
• Demonstrate at least 1 year of experience in LTC/SNF settings.
• Complete required immunizations, including COVID-19 vaccination or valid exemption documentation.
• Possess excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.
• Maintain current CPR and BLS certifications.
