Job Summary:
The Registered Nurse Non-Certified Case Manager specializes in coordinating care for patients by collaborating with healthcare teams and conducting thorough assessments. The role focuses on developing and monitoring individualized care plans to ensure optimal patient outcomes and efficient discharge planning.

Responsibilities:

• Conduct comprehensive assessments of patients within regulated timelines to determine eligibility for case management.
• Develop and implement personalized care plans in collaboration with patients, families, physicians, and healthcare teams.
• Monitor ongoing patient progress, evaluate care plan effectiveness, and document interventions and goal achievement.
• Manage a caseload for regular outreach, engagement, and care management to ensure continuous patient support.
• Promote integration of behavioral health and community resources to enhance continuity of care.
• Maintain documentation compliance and complete required education and training as per organizational and industry standards.

Qualifications:

• Possess Montana RN licensure and valid BLS certification through the American Heart Association.
• Minimum of 2 years of nursing experience, including utilization review expertise.
• Demonstrate strong clinical judgment and the ability to work in a fast-paced, stressful environment.
• Exhibit excellent verbal, written, and interpersonal communication skills.
• Show proficiency in computer applications, EMR systems, and standard office equipment.
• Ability to analyze, organize, and prioritize multiple tasks effectively.
