Job Summary:

Seeking a skilled Physical Therapist to provide home health therapy services. The clinician will assess, treat, and develop care plans for adult patients in their homes, ensuring quality care and documentation adherence.

Responsibilities:

• Evaluate patient condition and develop personalized treatment plans.
• Conduct therapy sessions and administer interventions in patients’ homes.
• Document patient progress and treatment outcomes accurately within designated timeframes.
• Communicate effectively with healthcare team and family members regarding patient needs.
• Adhere to all agency, state, and federal regulations related to home health therapy.
• Maintain reliable transportation and ensure all documentation complies with agency standards.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess at least 1 year of recent adult home health therapy experience.
• Demonstrate proficiency in home health documentation systems, preferably HomeCare HomeBase.
• Be CPR and BLS certified.
• Have reliable transportation, a current driver’s license, and liability insurance.
• Be willing to travel up to 75 miles from the primary work location.
