Job Summary:

Provide skilled physical therapy services to patients in a home health setting, focusing on restoring mobility and functional independence. Promote healing and prevent further injury through personalized treatment plans.

Responsibilities:

• Assess patient needs and develop individualized therapy programs.
• Deliver physical therapy interventions during home visits, ensuring patient safety and treatment effectiveness.
• Document patient progress and update treatment plans regularly.
• Educate patients and caregivers on exercises, safety precautions, and ongoing care.
• Maintain compliance with all clinical guidelines, policies, and state regulations.
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes.

Qualifications:

• Hold a valid Physical Therapist license in the state of practice.
• Minimum of 1 year of recent adult home health physical therapy experience.
• Proficient in OASIS and EMR systems, specifically HomeCare HomeBase.
• Excellent clinical assessment and communication skills.
• Ability to travel up to 75 miles from the assigned clinic.
• Reliable transportation and current driver’s license with agency-required liability insurance.
