Job Summary:
Our organization is seeking a dedicated Occupational Therapist to provide in-home therapy services within a home health setting. The therapist will be responsible for assessing, planning, and implementing treatment strategies to improve patients' daily living and functional skills.

Responsibilities:

• Evaluate patients' physical and cognitive abilities to develop personalized treatment plans.
• Deliver therapeutic interventions during home visits, focusing on improving motor skills, coordination, and daily functioning.
• Document patient progress and update care plans accurately within established timeframes.
• Collaborate with a multidisciplinary team to coordinate patient care and ensure optimal outcomes.
• Travel up to 75 miles between patient homes, including attending periodic office meetings.
• Maintain current licensure, CPR/BLS certifications, and auto insurance as required.

Qualifications:

• Hold a valid Occupational Therapist license in the state of practice.
• Possess at least 1 year of recent adult home health experience.
• Demonstrate proficiency in documentation and home health EMR systems such as HomeCare HomeBase.
• Be willing to travel up to 75 miles from the primary care location.
• Have reliable transportation, a valid driver's license, and agency-required liability insurance.
• Ability to work independently and efficiently manage a caseload of 5-6 patients daily.
