Job Summary:

Provide occupational therapy services to patients in a home health setting, focusing on improving functional abilities and independence through personalized treatment plans. The therapist will conduct home visits, documentation, and collaborate with care teams to meet patient goals.

Responsibilities:

• Evaluate patients' functional status and develop individualized treatment plans.
• Perform home visits, ensuring safety and adherence to therapy goals.
• Document patient progress and treatment activity accurately within required timeframes.
• Collaborate with healthcare providers and caregivers to optimize patient outcomes.
• Maintain compliance with all licensing, certification, and insurance requirements.
• Travel up to 75 miles from the assigned care center to patient homes, with mileage reimbursed.

Qualifications:

• Hold a valid state licensure as an Occupational Therapist in good standing.
• Possess at least 1 year of recent adult home health experience.
• Demonstrate proficiency in home health documentation and EMR systems, specifically HomeCare HomeBase.
• Have current CPR/BLS certification and, if applicable, on-call experience.
• Be willing to travel and provide reliable transportation, driver’s license, and auto insurance.
• Able to work independently with strong organizational and communication skills.
