Job Summary:
Provide nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Ensure safe, efficient, and compassionate support in accordance with regulatory standards and facility policies.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor residents' health status and document changes promptly.
• Collaborate with interdisciplinary team to develop and implement care strategies.
• Educate residents and their families on health management and care plans.
• Adhere to facility policies regarding infection control and safety protocols.
• Maintain proper documentation and ensure compliance with legal and regulatory requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC/SNF environments.
• Demonstrate excellent communication and interpersonal skills.
• Ability to prioritize and manage multiple residents’ needs effectively.
• Knowledge of medication administration and documentation procedures.
• Capable of working in a fast-paced, team-oriented setting.
