Job Summary:
The Home Health Registered Nurse provides comprehensive nursing care to patients in a home setting, ensuring adherence to clinical protocols, regulatory standards, and patient needs. The role involves patient assessments, developing care plans, and coordinating services to promote optimal health outcomes.

Responsibilities:

• Assess patient and caregiver needs using the nursing process to deliver quality care.
• Develop and communicate individualized, evidence-based care plans involving patients and caregivers.
• Serve as Case Manager, managing caseloads and visit schedules to meet productivity standards.
• Accurately complete and maintain all OASIS documentation in compliance with policies and regulations.
• Implement medical and nursing interventions related to complex wound care, IV therapy, and other high-tech skills.
• Educate patients and caregivers on medications, treatment options, discharge instructions, and safety measures.

Qualifications:

• Possess current RN licensure in accordance with state licensing requirements.
• Demonstrate experience with OASIS documentation and home health protocols.
• Proficient in assessment, care planning, and patient education within a home health setting.
• Knowledgeable about infection control standards and advanced wound care techniques.
• Able to coordinate multidisciplinary services and communicate effectively with team members.
