Job Summary:
Provides comprehensive home nursing care to patients based on physicians' orders, applying the nursing process with a focus on patient-centered outcomes. Coordinates and supervises home health aides and licensed practical nurses, ensuring quality care delivery in the patient's home setting.

Responsibilities:

• Coordinate patient care visits, including initial assessments, wound care, and follow-up visits.
• Supervise and evaluate home health aides and LPNs/LVNs to ensure compliance with care plans and agency policies.
• Document patient interactions and care activities accurately using HomeCare HomeBase (HCHB) within required timeframes.
• Travel up to 75 miles to patient homes, ensuring timely visits in accordance with schedule.
• Participate in periodic office meetings and maintain open communication with the Clinical Manager.
• Adhere to all safety protocols, including proper use of PPE and documentation requirements.

Qualifications:

• Current RN license in Georgia with no pending license restrictions.
• Minimum of 1 year of recent adult home health or hospice experience.
• Proficient in OASIS documentation (used within the last 6 months) or willing to complete retraining.
• Valid driver’s license, reliable vehicle, and agency-required auto insurance.
• Ability to travel up to 75 miles from the care center and work independently.
• Strong clinical evaluation, documentation, and communication skills.
