Job Summary:
Seeking a qualified Speech Language Pathologist to provide assessment and therapy services in a Skilled Nursing Facility. The provider will support patient communication and swallowing needs to improve quality of life.

Responsibilities:

• Evaluate patients’ speech, language, and swallowing functions to determine treatment plans.
• Develop and implement individualized therapy programs tailored to patient needs.
• Collaborate with healthcare team to coordinate care and track progress.
• Document patient assessments, treatment plans, and progress notes accurately.
• Educate patients and families on strategies to improve communication and swallowing.

Qualifications:

• Hold current certification and licensure as a Speech Language Pathologist.
• Possess previous experience in a SNF or similar setting.
• Demonstrate strong assessment and communication skills.
• Proficient in documenting clinical notes and treatment outcomes.
• Ability to work independently and as part of a multidisciplinary team.
