Job Summary:

Seeking a Speech Language Pathologist to provide expert speech therapy services within a home health setting. The role involves evaluating, treating, and supporting patients to improve communication and swallowing functions.

Responsibilities:

• Assess patients' speech, language, and swallowing capabilities to develop personalized treatment plans.
• Deliver direct therapy interventions to patients in their homes, focusing on improving communication skills.
• Document patient progress, treatment sessions, and clinical notes accurately in EMR systems.
• Collaborate with caregivers and healthcare teams to coordinate care and optimize patient outcomes.
• Manage a caseload of 5-6 patients daily, ensuring timely and effective therapy sessions.
• Maintain compliance with travel, documentation, and licensing requirements.

Qualifications:

• Hold a valid state licensure as a Speech Language Pathologist.
• Possess at least 1 year of recent adult home health or hospice experience.
• Demonstrate proficiency in HomeCare HomeBase, HomeBase Oasis, Outlook, or similar EMRs.
• Be willing to travel up to 75 miles from the assigned care center.
• Have current CPR and BLS certifications.
• Maintain reliable transportation, valid driver’s license, and liability insurance.
