Job Summary:
Provides professional nursing care to patients in the rehabilitation setting, supporting recovery and functional improvement. Collaborates with interdisciplinary teams to ensure safe, effective, and patient-centered care throughout the treatment process.

Responsibilities:

• Administer nursing interventions and monitor patient responses within the rehabilitation unit.
• Develop, update, and communicate comprehensive care plans tailored to individual patient needs.
• Educate patients and their families on condition management, recovery strategies, and home care instructions.
• Participate in multidisciplinary rounds to coordinate care and progress toward treatment goals.
• Ensure accurate and complete clinical documentation, including assessments and care activities.
• Promote a safe and therapeutic environment, adhering to safety protocols and advocating for patient well-being.

Qualifications:

• Hold a current valid RN license in New Hampshire.
• Demonstrate proficiency in basic dysrhythmia interpretation or complete training prior to orientation end.
• BLS certification required; CPI training preferred or completed before orientation.
• Graduate of an accredited nursing program preferred.
• Exhibit a strong understanding of rehabilitation patient needs and safety standards.
• Proven ability to collaborate effectively within interdisciplinary teams.
