Job Summary:

Provide comprehensive home health nursing services to patients requiring in-home care, ensuring adherence to regulatory standards and agency policies. Serve as a case manager, assessing patient needs and developing individualized care plans to promote optimal health outcomes.

Responsibilities:

• Assess patient and caregiver needs using the nursing process to deliver high-quality care.
• Develop and communicate personalized, evidence-based care plans including patient and caregiver involvement.
• Serve as Case Manager, actively managing caseloads and coordinating visit schedules to meet productivity standards.
• Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective patient care.
• Maintain up-to-date skills in high-tech nursing procedures such as IV therapy and wound care, ensuring infection control standards.
• Collaborate with LPNs and other disciplines, delegating tasks and reviewing documentation to ensure care quality.
• Evaluate the effectiveness of care plans, patient responses, and problem resolution to adjust interventions as needed.
• Document nursing care accurately and in a timely manner, ensuring comprehensive information sharing.
• Educate patients and caregivers on medication, treatment options, discharge instructions, and reportable conditions.
• Coordinate interdisciplinary services, including physical therapy and social work, to address comprehensive patient needs.
• Complete necessary agency, state, and federal documentation promptly, including Medicare forms and notices.
• Conduct visits efficiently, coordinating with schedulers and avoiding unnecessary combination of visits.
• Assess needs for HCA services, develop care coordination plans, and supervise ongoing care.
• Report patient incidents (infections, injuries) to QI specialists promptly.
• Manage safe discharges and transfers, ensuring all documentation complies with policies and regulations.
• Coordinate patient care with healthcare providers and vendors to support safe and effective outcomes.
• Participate in monthly peer review audits, providing and receiving constructive feedback.
• Facilitate case conferences with team members and management for optimal communication.
• Advocate proactively for patient care issues, ensuring quality and personalized service.

Qualifications:

• Current valid Registered Nurse (RN) license in the state.
• Proven experience in home health or related nursing field.
• Strong knowledge of home health regulations, standards, and documentation requirements.
• Competency in high-tech skills such as IV therapy and wound management.
• Excellent communication and interdisciplinary coordination skills.
• Ability to manage multiple caseloads efficiently and independently.
