Job Summary:
Provide patient-centered occupational therapy services under the supervision of a licensed Occupational Therapist in a skilled nursing facility. Focus on enhancing residents' functional independence through personalized treatment plans and interdisciplinary collaboration.

Responsibilities:

• Implement occupational therapy treatment plans for short-term and long-term residents.
• Communicate effectively with the Occupational Therapist and care team regarding patient progress and goals.
• Maintain compliance with facility policies, Medicare guidelines, and regulatory standards.
• Complete accurate documentation in Net Health or applicable electronic health records.
• Adapt quickly to changing patient needs and facility protocols.
• Support interdisciplinary team efforts to optimize patient outcomes.

Qualifications:

• Active OTA License required.
• Prior SNF experience preferred; mentorship available for those with 0-1 year experience.
• Demonstrate strong documentation and time-management skills.
• Possess a flexible and collaborative mindset.
• Ability to work Sunday - Thursday or Tuesday - Saturday schedules preferred.
