Job Summary:
The Occupational Therapist evaluates patients' needs, establishes treatment goals, and implements intervention plans to promote independence in activities of daily living. They collaborate with healthcare teams and provide patient-centered care to enhance functional outcomes.

Responsibilities:

• Assess, plan, treat, evaluate, and implement individualized plans of care for all patients.
• Demonstrate understanding of admission procedures, treatment costs, billing, and financial support options.
• Practice in accordance with AOTA standards of care and ethical guidelines.
• Collaborate effectively with physicians and healthcare team members to coordinate patient care.
• Supervise and support OT aides and students to ensure quality clinical education.
• Maintain a clean, safe environment and adhere to error prevention practices.

Qualifications:

• Hold an active Illinois state license as an Occupational Therapist.
• Possess a minimum of 1 year of home health OT experience.
• Have current BLS certification (AHA) and COVID vaccination.
• Demonstrate proficiency with OASIS and start of care documentation.
• Provide two supervisory references and a skills checklist completed within the past year.
