Job Summary:
The Licensed Practical Nurse (LPN) position provides direct patient care, including medication administration, monitoring resident conditions, and collaborating with the healthcare team to ensure quality outcomes. This role supports nursing staff and assists in maintaining a safe, comfortable environment for residents.

Responsibilities:

• Administer medications to residents following the 5 Rights to ensure safety and compliance.
• Monitor and report any changes in residents' conditions promptly to the charge nurse and physician.
• Assist Certified Nursing Assistants (CNAs) with call lights and other resident care responsibilities.
• Document and communicate changes in resident status to family members, physicians, and team members as needed.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Perform additional duties as assigned to support overall resident care and facility operations.

Qualifications:

• Hold and maintain a valid Wisconsin LPN license or RN licensure.
• Current CPR/BLS certification required.
• Possess a minimum of 1 year of related healthcare experience preferred.
• Demonstrate exceptional accuracy and attention to detail in all tasks.
• Be open to candidates with either RN or LPN credentials.
• Have effective communication skills to update team members, residents, and family members.
