Job Summary:
Seeking a compassionate and experienced RN Case Manager to provide comprehensive home health care services. The role involves collaborating with patients, families, and interdisciplinary teams to develop and coordinate personalized care plans.

Responsibilities:

• Develop and implement individualized care plans in collaboration with physicians, patients, and caregivers.
• Monitor and reassess patient progress, updating care plans as needed.
• Coordinate patient care activities through effective communication with team members and case conferences.
• Educate patients and caregivers on self-care techniques, medication management, and health maintenance.
• Document clinical evaluations, treatment goals, and patient progress accurately.
• Communicate significant changes in patient conditions to physicians and team members promptly.

Qualifications:

• Possess a current RN license for Vermont and New Hampshire or have an unencumbered compact license.
• Graduate from an accredited nursing school.
• Bring a minimum of 2 years of experience in home health care.
• Demonstrate strong clinical assessment and communication skills.
• Ability to work independently within a collaborative team environment.
