Job Summary:
The Hospice Registered Nurse provides compassionate in-home care and emotional support to patients experiencing life-limiting illnesses. The nurse is responsible for coordinating patient care, educating families, and ensuring compliance with regulatory requirements.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Deliver direct nursing care in patients' homes or facilities.
• Educate patients and families on disease management and symptom control.
• Document all clinical activities and patient interactions electronically.
• Communicate effectively with on-call team members and healthcare providers.
• Support patients emotionally and provide respite for families as needed.

Qualifications:

• Hold a valid Virginia RN license.
• Possess a minimum of 1 year of hospice experience.
• Maintain a valid driver’s license with a good driving record.
• Demonstrate strong interpersonal and communication skills.
• Ability to work independently and manage multiple patient cases.
