Job Summary:
Seeking a qualified Physical Therapy Assistant (PTA) with home health experience to provide patient care in a home setting. The ideal candidate will possess current licensure, excellent clinical skills, and the ability to work independently within established protocols.

Responsibilities:

• Deliver high-quality physical therapy services to patients in their homes according to physician orders.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress and maintain accurate clinical records in compliance with agency policies.
• Collaborate with healthcare team members to optimize patient outcomes.
• Ensure adherence to all safety protocols and infection control standards.
• Complete required compliance documentation within designated timeframes prior to assignment.

Qualifications:

• Current South Carolina PTA license or eligible to obtain prior to start date.
• At least 1-2 years of home health physical therapy experience preferred.
• Ability to pass cardiac dysrhythmia and moderate sedation tests during orientation.
• Recent EPIC electronic health record experience preferred but not mandatory.
• Capable of working Mon-Fri from 8:30 AM to 5:00 PM.
• Must be able to travel within a 75-mile radius for patient visits.
