Job Summary:
Assists patients in their recovery process by providing skilled physical therapy support in a home health setting. Ensures treatment plans are effectively implemented to promote patient mobility and independence.

Responsibilities:

• Deliver quality physical therapy care to patients in their homes according to physician orders.
• Assess patient progress and modify treatment plans as needed to meet individualized goals.
• Educate patients and family members on home exercise programs and safety precautions.
• Document patient encounters accurately and in a timely manner using electronic health records.
• Collaborate with healthcare team members to coordinate care and optimize outcomes.

Qualifications:

• Hold a valid PTA license in South Carolina prior to start and obtain it if pending.
• Possess at least 1-2 years of recent home health care experience preferred.
• Demonstrate proficiency in EPIC or similar electronic health record systems (preferred but not required).
• Be able to pass Cardiac Dysrhythmia and Moderate Sedation tests during orientation.
• Meet all compliance documentation requirements 5 business days before scheduled start date.
