Job Summary:

The RN Home Health role coordinates and delivers skilled nursing care in patients’ homes according to physician orders, agency policies, and the nursing process. This position reports to the Clinical Manager and supports care continuity by overseeing Home Health Aides and Licensed Practical/Vocational Nurses.

Responsibilities:

• Coordinate and direct home health nursing services based on physician orders.
• Apply the nursing process to assess, plan, implement, and evaluate patient care.
• Oversee, mentor, and evaluate Home Health Aides and LPN/LVNs to support safe delivery of care.
• Perform wound care and follow-up visits to promote recovery and prevent complications.
• Document clinical findings and care activities accurately in required EMR systems.
• Participate in periodic office meetings and collaborate with the care team to ensure continuity.

Qualifications:

• Hold an active Registered Nurse (RN) license with license in hand.
• Possess a minimum of 1 full year of Adult Home Health or Hospice experience (based on specialty).
• Maintain current CPR/BLS certification.
• Demonstrate proficiency with HomeCare HomeBase (HCHB) used within the last 6 months (or complete retraining if needed).
• Demonstrate proficiency with Oasis (required for RNs) used within the last 6 months (or complete retraining if needed).
• Provide reliable transportation, a valid driver’s license, and required liability insurance, with willingness to travel up to 75 miles from the assigned care center.
