Job Summary:
Provides comprehensive home health nursing services to patients, coordinating care based on physician orders and agency policies. Responsible for direct supervision of home health aides and licensed practical or vocational nurses, ensuring quality patient care across various settings.

Responsibilities:

• Assess patients' health status and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Supervise and evaluate home health aides and LPNs/LVNs to ensure compliance with care standards.
• Document patient care activities accurately within required timeframes.
• Coordinate travel between patient homes, maintaining efficient scheduling.
• Participate in office meetings and adhere to agency protocols and compliance standards.

Qualifications:

• Hold a valid RN license in good standing.
• Possess a minimum of 1 year full-time adult home health or hospice experience.
• Proficient in HomeCare HomeBase (HCHB) and OASIS within the last 6 months.
• Have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Capable of traveling up to 75 miles from the home clinic.
• Available to work flexible shifts including day and on-call rotations.
