Job Details

Job Summary:
Provide skilled nursing care to patients in a long-term care or skilled nursing facility setting. Ensure patient safety, comfort, and well-being through administering treatments and supporting daily activities.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Perform skin assessments, wound care, and monitor patient conditions.
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, and mobility.
• Manage specialized equipment including nebulizers, wound vacs, and CPAP machines.
• Document patient care activities and communicate effectively with the healthcare team.
• Comply with facility policies regarding vaccination and licensing requirements.

Qualifications:

• Hold a valid Hawaii LVN or LPN license in good standing.
• Possess current CPR and Medical Assistant certifications.
• Demonstrate experience in long-term care or skilled nursing environments.
• Ability to perform assessments and manage multiple patient needs efficiently.
• Exhibit excellent communication and documentation skills.
• Be able to work varied shifts, including weekends and holidays.
