Job Summary:

Provides comprehensive home health nursing care by coordinating and delivering patient treatments according to physician orders, agency policies, and nursing best practices. Responsible for evaluating patient needs, implementing care plans, and supervising nursing assistants to ensure high-quality home-based care.

Responsibilities:

• Coordinate and conduct home health visits, including start of care, wound care, and follow-up assessments.
• Evaluate patient conditions, administer treatments, and update documentation accurately within required timeframes.
• Supervise and evaluate home health aides and licensed practical nurses/licensed vocational nurses to ensure compliance with care plans.
• Travel up to 75 miles from the assigned care center to provide patient care across the designated territory.
• Maintain proficiency in HomeCare HomeBase (HCHB) and Oasis, ensuring all documentation meets agency standards.
• Participate in scheduled meetings, on-call rotations, and occasional weekend duties as required.

Qualifications:

• Hold a valid RN license in the state of assignment with no pending licensing issues.
• Possess a minimum of 1 full year of recent adult home health or hospice experience.
• Demonstrate proficiency with HCHB EMR system within the last six months.
• Certified in CPR/BLS and maintain current certification.
• Have reliable transportation, valid driver’s license, and agency-required liability insurance.
• Be willing to travel up to 75 miles and accept the physical demands of home visits.
