Job Summary:

The Registered Nurse (RN) provides comprehensive home healthcare services by coordinating and delivering patient care according to physician orders and agency policies. They supervise home health aides and licensed practical nurses, ensuring quality care in patients' homes.

Responsibilities:

• Administer and coordinate nursing care for patients in their homes, following physician orders and established protocols.
• Supervise and evaluate the performance of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Document patient assessments, care plans, and progress within the required EMR systems, including HomeCare HomeBase and Oasis.
• Travel to patient homes within a designated territory, providing care and follow-up visits as scheduled.
• Participate in team meetings and collaborate with physicians, caregivers, and team members to optimize patient outcomes.
• Ensure compliance with state and federal regulations, including certifications such as CPR and BLS, and maintain current licensure.

Qualifications:

• Possess a valid, active RN license in the state of practice.
• Minimum of 1 full year of recent adult home health or hospice nursing experience.
• Proficient in Oasis documentation and experienced with HomeCare HomeBase EMR within the last 6 months.
• Have reliable transportation, valid driver’s license, and agency-required liability insurance.
• Ability to travel up to 75 miles from the assigned clinic location, managing long drives between patient appointments.
• Capable of working flexible schedules, including occasional on-call rotations and weekend shifts.
