Job Summary:

Seeking an experienced Occupational Therapist for home health services in Baltimore County/City. The therapist will provide patient-centered care to geriatric patients, ensuring quality outcomes in a compassionate home environment.

Responsibilities:

• Evaluate and develop personalized treatment plans for geriatric patients in a home health setting.
• Document patient progress and treatment data accurately using Oasis documentation standards.
• Collaborate with patients, families, and healthcare teams to optimize care delivery.
• Adhere to productivity goals, achieving 30 points per week.
• Participate in on-call duties and weekend coverage as required.
• Maintain compliance with all travel and licensing requirements.

Qualifications:

• Possess at least 2 years of home care experience.
• Demonstrate proficiency with Oasis documentation procedures.
• Hold a valid Maryland OT license.
• Have BLS certification and a reliable vehicle with a valid driver's license.
• Capable of working Monday through Friday, 8:00 AM to 4:30 PM shifts.
