Job Summary:
Provide comprehensive social services to patients and their families, supporting their emotional, social, and psychological well-being. Collaborate with healthcare teams to develop and implement individualized care plans.

Responsibilities:

• Assess clients' psychosocial needs and establish appropriate support strategies.
• Develop, document, and update care plans tailored to each patient's circumstances.
• Facilitate patient and family education regarding treatment options and community resources.
• Coordinate with multidisciplinary teams to ensure continuity of care.
• Advocate for patients to connect them with necessary services and resources.

Qualifications:

• Hold a valid state license as a Licensed Clinical Social Worker (LCSW) or comparable credential.
• Demonstrate experience in a healthcare or clinical setting working with diverse patient populations.
• Possess strong assessment and counseling skills.
• Exhibit excellent communication and interpersonal abilities.
• Maintain knowledge of community resources, legal regulations, and ethical standards.
