Job Summary:

Provide acute and rehabilitative physical therapy services to patients in a skilled nursing facility, ensuring optimal recovery and functional mobility. Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.

Responsibilities:

• Assess patients' physical conditions and diagnose mobility or functional issues.
• Develop and execute personalized treatment programs aligned with patient needs.
• Document patient progress and adjust therapy plans accordingly.
• Educate patients and families on exercises and interventions to promote recovery.
• Maintain compliance with regulatory standards and facility policies.

Qualifications:

• Hold a valid state licensure as a Physical Therapist.
• Possess proven experience in a skilled nursing or rehabilitation setting.
• Demonstrate strong assessment and interpersonal skills.
• Be committed to patient-centered care and recovery goals.
• Maintain current CPR and First Aid certifications.
