Seeking a qualified Physical Therapist to provide rehabilitation services in a skilled nursing facility. The therapist will assess patient needs, develop treatment plans, and deliver therapies to improve mobility and function.

Responsibilities:

• Evaluate patients to determine their physical therapy needs and develop personalized care plans.
• Implement therapeutic interventions to enhance mobility, strength, and overall health.
• Collaborate with nursing staff and administrative teams to ensure quality patient care.
• Document patient progress and adjust treatments accordingly to meet recovery goals.
• Educate patients and families on therapy routines and post-discharge care.

Qualifications:

• Valid state-issued Physical Therapist license.
• Proven experience working in a SNF, post-hospital, or rehab environment.
• Strong assessment and treatment planning skills.
• Excellent communication and teamwork abilities.
• Ability to adapt therapy approaches to individual patient needs.
