Job Summary:

Provide comprehensive nursing care to hospice patients in a home setting, focusing on pain management, symptom control, and emotional support. Serve as a key member of the interdisciplinary care team to ensure quality end-of-life care.

Responsibilities:

• Develop and implement individualized care plans in collaboration with physicians, patients, and families.
• Coordinate patient care through interdisciplinary communication and case reviews.
• Conduct ongoing assessments of clinical, functional, and emotional status to identify needs and adjust care plans accordingly.
• Educate patients and caregivers on symptom management and end-of-life care techniques.
• Document patient evaluations, responses to treatment, and progress toward goals.
• Communicate significant changes in patient condition to the healthcare team and primary care physicians.

Qualifications:

• Hold an active Registered Nurse license in Vermont and New Hampshire or an unencumbered compact license.
• Graduate from an accredited nursing program.
• Demonstrate experience in hospice, medical-surgical, or oncology nursing preferred.
• Possess strong communication and assessment skills.
• Ability to work independently while contributing to a team-based environment.
