Job Summary:
Provide specialized hospice nursing care to patients in a home care setting, ensuring comfort and quality of life for terminally ill patients. Manage caseloads effectively while adhering to hospice protocols and standards.

Responsibilities:

• Assess patient needs and develop individualized care plans aligned with hospice guidelines.
• Deliver compassionate nursing care in patients' homes, including symptom management and education.
• Collaborate with interdisciplinary teams to coordinate ongoing care and improve patient outcomes.
• Document all clinical interactions accurately within the McKesson/Netsmart EMR system.
• Conduct routine visits, monitor patient progress, and adjust care plans as necessary.
• Ensure compliance with all hospice and healthcare regulations and standards.

Qualifications:

• Hold an active RN license with recent hospice case management experience.
• Demonstrate experience providing hospice care in a home setting.
• Have prior experience with rural travel to serve patients in diverse locations.
• Be proficient with McKesson/Netsmart EMR systems.
• Possess excellent communication and compassionate patient care skills.
• Preferably have no RTO (Request for Time Off) restrictions during the contract period.
