Job Summary:
Provide comprehensive social work services to hospice patients and their families, supporting emotional, psychological, and social needs during end-of-life care. Collaborate with interdisciplinary teams to enhance patient comfort and facilitate smooth transitions through the hospice process.

Responsibilities:

• Assess patients’ psychosocial needs and develop individualized care plans.
• Provide grief and loss counseling to patients and family members.
• Coordinate resources and support services to meet patient needs.
• Educate families about hospice services, advance directives, and community resources.
• Maintain detailed documentation of patient interactions and interventions.
• Support interdisciplinary team communication and care coordination.

Qualifications:

• Hold a Bachelor's degree in social services, counseling, or psychology; Master's degree preferred.
• Possess a valid Driver’s License and auto liability insurance.
• BLS certification for healthcare providers is required.
• Demonstrate experience or education in grief and loss counseling.
• Recent hospice or healthcare setting experience preferred.
• Strong interpersonal skills and ability to work independently.
