Job Summary:
Provide comprehensive case management services to hospitalized patients, ensuring quality care coordination, optimal patient outcomes, and resource utilization. Utilize Epic and clinical expertise to facilitate patient progress and discharge planning.

Responsibilities:

• Assess patient needs and develop individualized care plans in collaboration with interdisciplinary teams.
• Document all patient interactions, assessments, and care plans accurately in Epic.
• Coordinate and communicate with healthcare providers, patients, and families to ensure seamless care transitions.
• Monitor patient progress and adjust care plans as necessary to meet medical and psychosocial needs.
• Ensure compliance with hospital policies, regulatory standards, and travel policies.
• Manage unit-specific protocols, including float requirements and credential verifications.

Qualifications:

• Hold a valid nursing license in the state of employment; pending licenses accepted with in-hand verification the Wednesday before start date.
• Minimum of 1-2 years of inpatient acute care hospital case management experience.
• Demonstrate proficiency with Epic electronic medical records system.
• BSN degree preferred; RN license required.
• Ability to adhere to RTO policies, float requirements, and travel compliance regulations.
