Job Summary:

Provides comprehensive home nursing care to patients according to physician orders, utilizing the nursing process to ensure quality care delivery. Supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses to maintain clinical standards.

Responsibilities:

• Coordinate and deliver individualized nursing care during home visits in accordance with agency policies.
• Assess patient conditions, document findings, and develop tailored care plans.
• Supervise and evaluate home health staff, including aides and LPNs/VNs.
• Ensure timely and accurate documentation of patient visits using HomeCare HomeBase (HCHB), including proficiency in Oasis for applicable cases.
• Travel up to 75 miles from the home clinic to cover assigned territory.
• Participate in periodic team meetings and adhere to scheduled hours, including potential weekend shifts or on-call rotations.

Qualifications:

• Hold an active, valid RN license in the relevant state.
• Possess at least one full year of adult home health or hospice experience in a recent clinical role.
• Proficient in HomeCare HomeBase (HCHB) EMR system within the last 6 months; Oasis certification preferred.
• Have reliable transportation, valid driver’s license, and agency-required liability insurance.
• Demonstrate strong clinical assessment, documentation, and supervisory skills.
• Able to travel up to 75 miles and work independently in a home health setting.
