Job Summary:

Provides bedside nursing care to home health patients by coordinating and implementing physician orders based on agency policies and procedures. Responsible for supervising home health aides and practical nurses while ensuring quality patient care in a home setting.

Responsibilities:

• Coordinate and deliver skilled nursing care to patients in their homes according to physician plans.
• Supervise and evaluate home health aides, licensed practical nurses, and licensed vocational nurses.
• Document patient assessments, care plans, and progress accurately in the electronic health record system.
• Travel to patient homes within a designated territory, managing a daily caseload of 5-8 patients.
• Participate in periodic office meetings and on-call rotations as required.
• Ensure compliance with agency policies, documentation standards, and state licensure requirements.

Qualifications:

• Valid nursing license in good standing in the state of practice.
• Minimum of 1 year of recent adult home health or hospice nursing experience.
• Proficient in HomeCare HomeBase (HCHB) EMR system; experience with Oasis strongly preferred.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong assessment, communication, and patient education skills.
