Job Summary:
Seeking a highly experienced Registered Nurse for inpatient case management within a hospital setting. The role involves discharge planning, complex patient coordination, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Responsibilities:

• Manage a caseload of 15-20 acute inpatient patients daily, coordinating discharge plans and care transitions.
• Collaborate with Social Work, Pharmacy, and RT teams to facilitate seamless patient discharges.
• Review patient charts, develop individualized discharge plans, and ensure timely execution.
• Meet criteria for complex placements, including financial assistance eligibility for uninsured or underinsured patients.
• Adhere to hospital policies, including vaccination, fingerprinting, and dress code requirements.
• Quickly assimilate minimal unit training and work efficiently within a fast-paced, high-acuity environment.

Qualifications:

• Possess a minimum of 3 years of acute inpatient case management experience in a large hospital (500+ beds, Level 1/Academic Medical Center).
• Hold a valid RN or BSN degree; ADN candidates considered.
• Demonstrate strong computer skills and experience with Epic EMR systems.
• Maintain an active BLS certification through AHA; CCM preferred but not required.
• Have recent inpatient discharge planning experience within the last two years.
• Ability to work independently and adapt quickly to a dynamic healthcare environment.
