Job Summary:

Provides comprehensive home health nursing care by assessing, planning, implementing, and evaluating patient care plans in accordance with physician orders and agency policies. Manages a caseload of patients, supervising home health aides and licensed practical nurses/laboratory nurses to ensure quality care delivery.

Responsibilities:

• Coordinate and deliver skilled nursing care to patients in their homes, following physician orders and established protocols.
• Supervise and evaluate the performance of home health aides and LPNs/LVNs, ensuring compliance with care standards.
• Document patient encounters accurately and within required timeframes using HomeCare HomeBase and Oasis EMR systems.
• Travel up to 75 miles per day to conduct patient visits, including start of care, wound care, and follow-up assessments.
• Maintain current licensure, CPR/BLS certifications, and meet on-call requirements as specified.
• Collaborate with the clinical team to develop and update care plans tailored to individual patient needs.

Qualifications:

• Possess a valid RN license in the state of practice; pending licenses are not accepted.
• Minimum of 1 year of recent adult home health or hospice experience, or equivalent specialty experience.
• Proficient in using HomeCare HomeBase EMR; OASIS proficiency required for RNs.
• Demonstrate excellent documentation and communication skills in a home health setting.
• Ability to travel and work independently, with reliable transportation and a valid driver’s license.
• Willingness to work flexible hours, including weekends and on-call rotations as required.
